The Digital Transformation Maturity Assessment Model is a questionnaire designed to evaluate an individual's level of digitalization, with the goal of facilitating the implementation of an effective digital transformation process.

On one hand, this diagnostic tool is oriented toward aiding management decision-making, enabling the assessment of the company's digital maturity. Similarly, it serves as a supportive tool for the improvement process by identifying critical areas for the digital transition.

On the other hand, the tool is directed at individuals, allowing them to assess their digital and soft skills necessary for implementing a digital transformation in their daily life. 

Given the scope of the Digital Transformation Maturity Assessment Model, this diagnostic tool can be utilized by CEOs, managers, employers, workers, entrepreneurs, students, unemployed individuals, or anyone seeking to evaluate their skills necessary for implementing an effective digital transformation process. 

This tool was developed within the framework of the project "Small.Com - Empowering Small Businesses for Sustainability with Digital Transformation", under the Erasmus+ Programme.
